Healthcare Coordinator
Job Description
The Healthcare Coordinator is a registered nurse with oversight for maintaining and implementing positive healthcare services for frail older adults at Virginia Tech's Adult Day Services. The nurse will model best practices for staff and students and is responsible for administering, monitoring, and evaluating health care needs of all center participants, including medication administration and care plans. He/she must communicate effectively with participants and families as well as the interdisciplinary team, regulators and students. He/she must complete necessary documents required by all regulatory agencies, i.e. Medicaid, Veterans Affairs, DSS and protocols for service administration. The Healthcare Coordinator contributes to oversight, training, and scheduling of all direct care staff.
Required Qualifications
The Healthcare Coordinator is a registered nurse (RN). The nurse must be licensed to practice nursing in the Commonwealth of Virginia. He/she must have two years of related clinical experience which may include working in an acute care hospital, rehabilitation hospital, or nursing home. In addition, this individual must also maintain a satisfactory work record and be able to lift 50lbs. Computer skills are necessary for this position.
Preferred Qualifications
Preferred qualifications include experience working in dementia care setting, and experience with staff supervision and training.
