OVERVIEW
Same Day Access (SDA) allows a person in need of services the ability to call or walk in for an initial assessment - often on the same day. SDA eliminates the need for that individual to be placed on a waiting list, thereby connecting them with services in a timelier manner. This process includes admission paperwork, brief evaluation, health screening, crisis planning, and if recommended and appropriate, a full Comprehensive Needs Assessment.
The role of an SDA Case Manager is to provide support and follow up to the SDA assessment team and overall process. Case manager will provide daily collaboration and coordination with internal and external providers regarding follow up on consumers who present to the SDA process at both the Montgomery and Radford Clinics. The SDA Case Manager will provide services and additional referrals for services based on individual and family needs. Additionally, the case manager may assist consumers in navigating resources and other tasks such as linking to financial, housing, and other resources.
The case manager may be required to provide some limited community case management but will primarily be working in the clinic where SDA is scheduled. This position will require flexibility, excellent time management skills, organization, and ability to multi-task. All other duties as assigned will be required.
QUALIFICATIONS
Graduation from an accredited college or university with a Bachelor's degree in Social Work or human services related field with at least one year of experience in a setting working with children or adults with needs in the areas of mental health, substance use, or developmental disability preferred. An appropriate applicant will demonstrate knowledge of case management/social work practices and resources available to help consumers in the community. QMHP status preferred. Valid Virginia driver's license with a safe driving record required.
