Mental Health Case Manager
Goochland Powhatan Community Services is seeking a Mental Health Case Manager to provide services to adults with serious mental illness.
GPCSis located 15 minutes off route 288 and serving the communities of Powhatan and Goochland. GPCS Mental Health Case Management is a small but mighty team, comprised of 5 dedicated case managers and a supervisor providing quality services to consumers in this area.
This position works closely with the nurse and psychiatric providers within the agency and as well as community providers. Duties include completing psychosocial needs assessments, developing Individual Service Plans (ISP), collaborating with collateral partners, assisting in medication management, providing education, scheduling appointments, making referrals as needed,documenting in an electronic health record, coordinating services, and monitoring medication compliance. The qualified candidate must have good oral and written communication skills and strong interpersonal skills. Computer literacy is required to be successful in this position as this agency uses Credible electronic health record.
Due to COVID-19, services are currently being provided primarily via telehealth for the safety of both consumers and employees. This position is provided a laptop to allow for working from home during this pandemic. The interview for this position will also take place via Zoom.
Bachelor's degree required. Bachelor's or Master's in a Human Service field is preferred. The candidate must meet the qualifications for Qualified Mental Health Case Manager. One to two years related experience in case management and working with the severe mental health illness population.
GPCS provides a comprehensive benefits packages including an option for free insurance and participation in the Virginia Retirement System. Salary starts at $38,002 based on experience and education.
