The RN Case Manager in our PACT Team (Program of Assertive Community Treatment) nursing and intensive case management services for adults with serious mental illness, substance abuse, or possibly co-occurring diagnoses of mental illness and substance abuse.
The RN Case Manager is responsible for writing and implementing individual treatment plans to ensure clients receive necessary services and meet their goals, following medication policies to ensure treatment protocol as prescribed by psychiatric nurse practitioner or psychiatrist is followed, and managing the system for getting medications to clients. The RN Case Manager manages pharmaceuticals and medical supplies to meet regulations, conducts comprehensive assessments of physical health, dental health, and use of drugs and alcohol. The RN Case Manager maintains a case load and assist the treatment team with managing clients' complex medical problems in addition their mental illnesses. Schedule includes rotating evenings, weekend and holiday hours to assist clients in receiving medications and maintain fidelity to the PACT model. This position requires local travel, up to 20%.

Our Program of Assertive Community Treatment (PACT) is individualized community-based treatment, rehabilitation, and supports designed to meet the needs of adults with serious mental illness who often require 24 hour service availability to avoid hospitalization. Complete wrap-around services include intensive case management, psychiatric diagnosis and treatment, medication prescription, administration and monitoring, Substance Abuse services, counseling, vocational counseling, and life skills training. Minimum Requirements:
1. RN License: Registered Nurse (active, unrestricted license in the Commonwealth of Virginia)
2. Preference for a work setting that requires multi-tasking and changing priorities.
3. Experience: at least one year nursing experience combined with experience in a psychiatric setting.
4. Driver's license: Active, unrestricted driver's license and a good driving record (no more than two moving violations in the last three years and no criminal violations within the last five years).
