Licensed Practical Nurse (LPN) INTERNAL RELIEF

The schedule for this position is 5-6pm on Tuesdays and Wednesdays
The LPN in Project LINK coordinates the care of patients receiving Medication Assisted Therapy (MAT) for opioid addiction via telehealth. They are responsible for monitoring health status, treatment progress, and medication adherence of the MAT patient.
The LPN works as a member of a multi-disciplinary team consisting of the physician, clinicians, therapists, peer specialists, and case managers. They meet with clients before their MAT appointment to take vital signs, complete the "clinical opioid withdrawal scale assessment, and document early withdrawal symptoms. They gather pertinent information for the electronic health record and discuss client concerns. Additional responsibilities include:
- Collect urine drug screens, vital signs, alcohol breath analysis
- Monitoring lab results
- Assist with activities required for assessment, treatment planning, and discharge of clients
- Documenting all pertinent nursing notes in the electronic health record.
Qualifications:
- Must have an active, unrestricted nursing license as an RN or LPN in the State of Virginia.
- Requires a minimum of one year of nursing experience.
- Familiarity with addiction/recovery and behavioral health.
