Housing Case Manager, Women's Services
The Housing Case Manager assists pregnant women and mothers with substance use disorders to access needed supports for maintaining stable affordable housing in the community. The Case Manager helps participants through a trauma informed lens to identify their strengths and barriers to stability, and works with them to develop individualized action plans that promote self-sufficiency. Networking skills are essential, as the Case Manager will outreach and engage individuals that meet criteria for services. They offer community-based treatment and rehabilitative services to ensure timely access to health care, reduce involvement with the child welfare system, promote family preservation, and positive pregnancy outcomes.
This position is grant based and is intended to provide services that enable women to ensure successful tenancy and reduce relapse/medical problems in order to live independently.

Requirements:
- Experience: One year of experience working with individuals with substance abuse is required
- Education/Credentials: Bachelor's degree in a human services field is required
- Driving Record: This position requires transporting clients and driving agency vehicles. Candidates must have an unrestricted driver's license and a good driving record (no more than two moving violations in the last three years AND no criminal violations in the last five years).
