Case Manager, Medicated-Assisted Treatment (MAT)
The Case Manager provides services for clients identified with an Opioid Use Disorder or Alcohol Use Disorder in the local jails who may benefit from participation in Medicated-Assisted Treatment (MAT). The Case Manager will educate inmates about MAT services and help them begin the medication before leaving the jail as well as during the transition to the community. This position is funded by the Medicated Assisted Treatment Grant.
Additional activities include:
- providing education to jail staff about MAT services
- discharge planning services
- coordination of care with MAT medical provider
- attend court
- connecting clients to available resources.

The successful candidate will have knowledge of community resources available and the criminal justice system, be comfortable working independently in a controlled environment, and be a team player.

Minimum Qualifications:
1. Degree: Bachelor's Degree the a human services field (psychology, sociology, social work, or similar).
2. Experience: One year of experience providing direct services to individuals with a diagnosis of mental illness or substance abuse
3. Background Checks: Candidates must be able to pass background checks including those required by the local jails.
4. Good driving record: Candidates must have a good driving record (no more than two moving violations in the last three years AND no criminal violations in the last five years (including DUI) and are required to undergo a fingerprint background check.
