Case Manager, Coordinated Specialty Care
Are you a case manager with strong leadership skills that has experience working with adolescents and or young adults?
The Case Manager Coordinated Specialty Care serves an individual and their family in accessing needed medical, psychiatric, social, educational, residential, and other supports essential for living in the community and meeting basic needs. This case management position will be a member of the Coordinated Specialty Care (CSC) team for the treatment of individuals with first episode psychosis.
The schedule for this position is Monday-Friday (no weekends)!
Minimum Requirements:
- Education: Minimum of a high school diploma. An Associate's or Bachelor's degree in a human services field preferred.
- Experience: One (1) year related experience working with adolescents and or young adults.
- Licensure/Certifications: Qualified Mental Health Professional (QMHP) preferred.
- Safe Driving Record: This position requires transporting clients or driving an agency vehicle. Candidates must have a valid Virginia driver's license with a safe driving record (no more than two moving violations in the last three years AND no criminal violations in the last five years).
